Job description
Maternal, Adolescent & Child Health Institute NPC, (MatCH) is a non-profit organisation supporting public sector health services. MatCH implements donor funded projects to improve HIV and TB outcomes through providing technical assistance and direct service delivery support. MatCH is looking for suitably qualified candidates to fill the following positions under a project aimed at Achieving and Sustaining HIV/TB Epidemic Control in the KwaZulu Natal Province
Location: King Cetshwayo District
Type of Contracts: Fixed Term Contracts ending 31 December 2026

Main purpose of the job
The incumbents will work as part of a roving community team to conduct adherence support, tracing, PrEP continuation, and mother–baby follow-up.
Key Responsibilities
· Provide support for adherence activities for adults and children on ART
· Implement patient tracking for pregnant and breastfeeding women on PrEP and ensure follow up for PrEP continuation
· Follow up on mother-baby pairs to ensure the management of HIV exposed infants
· Provide technical assistance on the management of patient tracking and tracing
· Ensure adherence and follow up conducted in line with Department of Health policies and procedures
· Strengthen recordkeeping on patient follow up and retention
· Conduct quality improvement assessments and implement remediation plans to address gaps in service delivery
· Meet with facility teams to coordinate the management of missed appointments, patient tracking and referrals
· Provide training and mentoring to Department of Heath staff on patient follow up and adherence support
· Implement Transition Readiness Assessments on patient tracing and retention
· Provide weekly, monthly and quarterly reports on patient follow up and retention in care
Minimum Qualifications and Experience
· Enrolled Nursing Qualification
· Registration with SANC
· 2 years’ experience in primary health care in public health setting
· Fluent in isiZulu
· Valid driver’s license and willingness to travel
Desirable additional education, work experience and personal abilities
· Experience in donor funded organisation and with PEPFAR
· Experience working in a multidisciplinary team
· Excellent facilitation skill
· Written and oral communication skills
Available to commence work immediately
