VACANCY: Locum Social Work Auxiliary (SWA)

Mitchells Plain Transitional Care Facility (MPTCF) - Managed by Aquarius Health Care NPC/NPO.

Mitchell's Plain Intermediate Care Facility is situated on the premises of Lentegeur Psychiatric Hospital. The facility has 148 beds, responsible for the rehabilitation of patients, wound care, palliative and convalescent care.

Job Purpose:

To, under supervision, provide a holistic clinical service including the following:

• Psychosocial support assisting families and patients to cope with acute, chronic or terminal illness.
• Arranging financial assistance through SASSA and GIA applications.
• Assisting patient to transition into the domiciliary environment or other place as determined by the patients care needs.

Key Requirements:

• NQF 5 certification as social work auxiliary
• Registration with SACSSP
• Experience in Rehabilitation: Patients with CVA, TBI, Wounds, Orthopaedic conditions
• Understanding of the various Therapy/Rehab MDT members: Occupational Therapy, Physiotherapy, Speech Therapy etc.
• Able to communicate in 2 of the 3 official languages of the Western Cape
• Drivers licence
• Police Clearance – not older than 3 months

Key Responsibilities:

• Assess and determine discharge plan
• Provide emotional support and counselling where indicated (within limitation of SWA scope)
• Assisting families with application for long-term placement
• Application for SASSA, GIA, ID

• Ability to work within a multi-disciplinary team
• Able to work under pressure in hospital environment.
• Excellent time management
• Computer and numeric skills
• Able to manage stress

ADDITIONAL TASK

• In addition to the above tasks, the official has to perform any other task (s) not mentioned above as long as it is within his/her capabilities and scope of work.
