📢 Job Opportunity: Professional Nurse – Home-Based Care 🌿🏡

We are seeking dedicated and compassionate Professional Nurses to join our Home-Based Care team. This role is ideal for skilled nurses who is passionate about community health and providing quality care to individuals in the comfort of their homes.

Key Responsibilities:

✅ Provide clinical oversight and support to Community Health Care Workers (CHWs).

✅ Conduct health assessments and develop individualized care plans.

✅ Administer treatments, medication Adherence, and wound care as needed.

✅ Educate patients and their families on disease management and preventive care.

✅ Ensure compliance with health and safety protocols.

✅ Maintain accurate medical records and report on patient progress.

Requirements:

✅ Registered with the South African Nursing Council (SANC).

✅ Experience in community health, home-based care, or primary healthcare.

✅ Strong communication and leadership skills.

✅ Ability to work independently and as part of a multidisciplinary team.

✅ Passion for patient-centered care and health education.

✅ A Valid drivers licence with own vehicle
