We seek to appoint a Professional Nurse, motivated and passionate to assist with the effective implementation of integrated wellness services at the community level, with focus on the Home and Community Based Care program in Langebaan and Hopefield. Position is effective from 01 October 2026.

Education, experience and competencies required:

· Nursing Qualification

· SANC Registered

· Provincial Guidelines

· Infection control protocol

· Use of CBS electronic referral system

· Computer literate

· Excellent writing skills

· Good client and partner relations

· Good use of language

· Confidentiality at all times

· Knowledge of Primary Health Care

· Driver’s license

· NIMART qualification will be to your advantage

· PACK 2025

· Must reside in the Western Cape

Duties and responsibilities

· The effective management and execution of relevant Curative Programmes within the scope of practice and to be an advocate for the patient to ensure the provision of necessary health care.

· The effective management and execution of relevant Child Health within the scope of practice and to be an advocate for the patient to ensure the provision of necessary health care.

· The effective management and execution of relevant Woman’s Health Services within the scope of practice and to be an advocate for the patient to ensure the provision of necessary health care.

· The effective management and execution of relevant HAST programmes within the scope of practice and to be an advocate for the patient to ensure the provision of necessary health care.

· Assessing the performance and effectiveness of programs, projects, and policies.

· Compiling monthly, quarterly and yearly reports.

· Providing leadership to a team of 15 Community Health Care Workers.

· Breaking down the project into smaller, manageable tasks allows for better organization and resource allocation. This structured approach helps in estimating timelines, costs, and resource requirements.
