Employment Type: Full-Time
Working Hours: U.S. Business Hours (EST) | 8am - 5pm
Budget: R25-30K CTC per month
About the Role
We are recruiting on behalf of a leading U.S.-based healthcare organisation seeking an experienced Registered Nurse to join their Clinical Intake team. This is a highly analytical nursing role that combines clinical expertise with administrative precision.
Rather than providing direct patient care, you will be responsible for reviewing incoming referrals, assessing patient eligibility for home healthcare services, and ensuring each patient is guided to the most appropriate level of care. You will play a critical role in the patient's journey by making informed clinical decisions, coordinating with healthcare professionals, and managing referrals from intake through to discharge.
This position is ideal for a nurse who enjoys using clinical judgement, thrives in a fast-paced environment, and is passionate about ensuring patients receive the right care at the right time.
Key Responsibilities
- Review and evaluate incoming patient referrals received via electronic referral platforms, hospitals, physicians and healthcare facilities.
- Assess clinical documentation to determine eligibility for home healthcare services.
- Evaluate patients against U.S. home health regulations, including:
- Homebound status
- Skilled nursing or therapy needs
- Determine whether referrals meet acceptance criteria based on clinical guidelines and insurance requirements.
- Identify patients who may benefit from palliative or hospice services and facilitate appropriate internal referrals.
- Manage each referral throughout the patient lifecycle, from initial intake through to discharge.
- Maintain accurate documentation and update internal systems with exceptional attention to detail.
- Collaborate with hospital case managers, care coordinators, physicians and internal clinical teams to obtain additional clinical information where required.
- Exercise sound clinical judgement while following established processes and compliance standards.
- Escalate complex or ambiguous cases appropriately while confidently managing routine referrals independently.
What We're Looking For
We are looking for a Registered Nurse who combines strong clinical knowledge with exceptional organisational skills.
The ideal candidate will possess:
- Current Registered Nurse qualification.
- Several years of nursing experience with strong exposure to adult and geriatric patient populations.
- Experience within home healthcare, case management, utilisation review, discharge planning, hospice, palliative care, oncology, medical-surgical nursing or similar clinical environments will be highly advantageous.
- Ability to confidently interpret clinical records, diagnoses, hospital documentation and care plans.
- Excellent understanding of complex medical conditions and disease progression.
- Strong critical thinking and problem-solving abilities.
- Exceptional organisational skills with the ability to prioritise multiple referrals simultaneously.
- High level of accountability and ownership.
- Outstanding attention to detail.
- Ability to work independently while knowing when to seek clinical guidance on complex cases.
- Excellent written and verbal English communication skills.
